Where does Subee work?

7 Steps to Support at Home
Accessing ongoing, long-term support services available to you with the Support at Home Program.

Interview
An assessor from My Aged Care (usually a social worker or nurse) will visit your home and discuss how you manage daily tasks. During the interview, you’ll decide together which home care services will best support you to be comfortable and safe at home. This interview usually lasts 1-3 hours.
Income Assessment
Services Australia, or the Department of Veteran’s Affairs, will then assess your income from sources like pensions & superannuation to decide how much you need to pay towards your care.
Approval Letter
My Aged Care will send you a letter advising your Support at Home reference number, your approved level of care, the types of services you can receive and the Government contribution to your care.
Funds Allocated
You’ll receive a letter from My Aged Care confirming you can start subsidised services. You have 56 days from assignment to choose a provider and sign a home care agreement.
Sign Agreement
The final step is signing your agreement with your services provider. A Home Care Agreement is a formal written contract between your provider and you detailing how your care services will be delivered.

Applying for Support at Home Program funding
Are you eligible? What does an assessment involve?

Eligibility
Should I Apply?
Eligibility for government-subsidised aged care services is based on your age, needs and situation. An aged care assessment will determine if you are eligible for services and – if you are – which program you’re eligible for. Scan the code for more information.
myagedcare.gov.au/should-i-apply
Preparing for an Assessment
To access this funding, My Aged Care will carry out an assessment to determine the level of support you need.
There are a few ways to apply for an aged care assessment:
Apply online – https://www.myagedcare.gov.au/apply-online
On the phone – Apply on the phone by calling My Aged Care on 1800 200 422.
Face-to-face – Talk to someone face to face. Book this by calling 1800 227 475.
Understanding what you will pay
The government pays for a percentage of the cost of each service, and recipients contribute the rest. How much you need to contribute will be determined by an income assessment conducted by Services Australia or the Department of Veteran’s Affairs.
